Your Life Care Partner

Seeking care for yourself or a loved one can be a daunting physical and emotional process. At Mozaic Concierge Living, we want to ensure you have support every step of the way through the work of our Life Care Coordinators. Expertly trained and here for all your needs, our Life Care Coordinators are tasked with helping make the transition to higher levels of care when needed. Our coordinators will work to create a multidisciplinary team, including your physicians, onsite staff, and key family members, to ensure a resident’s plan of care meets their unique needs. After making the move, coordinators remain connected to the resident, helping them transition to a new environment smoothly. In the event that a resident’s needs cannot be met on campus, Life Care Coordinators are available to help create a comprehensive plan and provide connections within the greater Stamford area.
